Demonstrate the types and uses of form elements. Explore the uses of check boxes, radio buttons, textboxes etc.
Demonstrate the filing of forms offline. Guide learners to open and save a page. Lead them to fill the forms offline.
Demonstrate uploading of files. Lead learners to explore the upload button by adding pictures, audio, pdf etc.

  • 2Ask learners: When you sign up for a school online portal or fill a registration form on your phone, what different types of boxes or buttons do you click? Learners whisper answers to their partner
  • UNDERSTANDING FORM ELEMENTS AND THEIR USES
  • 1Open a real online form on the projector using Internet connection (e.g., Google Form or school registration page). Point to a textbox and explain: This is a textbox—you type your name or address here. Point to a checkbox and explain: This is a checkbox—you can tick many boxes if more than one answer is correct. Point to a radio button and explain: This is a radio button—you can only pick ONE answer. Ask pairs: Which type would you use to enter your full name? Learners discuss and answer aloud
  • 2Distribute the Textbook or Learners Resource Pack showing labelled diagrams of form elements. Learners in pairs identify and circle three textboxes, two checkboxes, and one radio button on the printed page. A volunteer from the first pair to finish comes to the board and sketches one textbox shape while saying its purpose aloud
  • 3Call on learners in turn: Kofi, when you buy credit on MTN online, what form element do you use to enter your phone number? Akosua, if a form asks 'Select all subjects you like', is that a checkbox or radio button? Why? Learners respond; affirm correct answers and correct misconceptions using the projector example. Let learners work in pairs to keep all learners involved

  • 1Pairs compare their identified form elements from the textbook page with the adjacent pair. One representative from each pair reports one element type and its use to the class

  • 1Name the type of form element you would use to enter your date of birth in an online registration form, and explain why that element is correct in their exercise books.

  • 1Recall the steps for saving a webpage to a computer for offline use
  • 2Ask learners: Yesterday you visited a form online. Today, the internet is down. How do we still fill that form? Take 3 volunteer answers
  • SAVE A WEBPAGE OFFLINE USING THE COMPUTER
  • 1Open a sample form webpage on the projector with the internet connection active. Click File → Save As. Name the file 'GhanaForm.html'. Show learners where the file saves on the Desktop. Have them follow on their own computers/laptops simultaneously. Ask one learner to read aloud the filename saved. Let learners work in pairs to keep all learners involved
  • 2Disconnect the internet. Ask: Can we still see the form? Open the saved GhanaForm.html file from the Desktop. Learners confirm the form appears without internet. Pair learners: one reads the form fields aloud, the other points to each field on their screen
  • 3Guide learners to fill one form field (e.g. Name: 'Kofi', Age: '12'). Demonstrate typing in the text box. Ask: What happens when you type offline? Learners discuss with their partner whether the form accepts input without saving to a server

  • 1Learners stand. Ask: Show thumbs up if you successfully saved your webpage as an HTML file. Thumbs down if you need help. Scan the room and note support needed
  • 2Invite one learner to the projector: open the saved form file and fill in one field. Class watches and confirms the form works offline
Exercise
  • 1On your computer, save the form webpage provided by the teacher, disconnect the internet, open the saved file, and fill in the Name and Email fields, then show your work to a partner in their exercise books.

  • 1Identify the upload button and file types that can be added to online forms
  • 2Show learners a projected image of an online form on the screen and ask: Where do you click to add a picture to this form?
  • EXPLORING THE UPLOAD BUTTON
  • 1Using the computer and projector, demonstrate clicking the 'Choose File' or 'Browse' button on a sample Google Form; show learners the file browser window that opens and name three file types:.jpg (picture),.pdf (document),.mp3 (audio). Let learners work in pairs to keep all learners involved
  • 2Learners work in pairs on their laptops to locate and click the upload button on a prepared online form link provided in the textbook activity sheet; pairs report which file types they see in the dropdown menu
  • 3Call on one representative from each group of four to demonstrate opening the file browser on their computer and reading aloud one file type they found; confirm all answers using the projector to display the browser window

  • 1Learners open an online form on their computer, locate the upload button, click it, and name one file type shown in the file browser window in their exercise book
